MCC & HFA: Family Support Through Evidence Based Home Visiting
Healthy Families America (HFA) is a nationally recognized, evidence-based home visiting model specifically designed to work with families who are at risk for negative childhood experiences, including child maltreatment and abuse.
Ranging from light touch to intensive, MCC’s home visiting models pair families with community health workers (CHWs) who provide education on child health, development, and parenting, early learning support, and referrals for health-related social needs. Our objectives are to reduce incidences of child maltreatment and neglect by offering supportive interventions to families building protective factors and connection to resources.
MOMobile® programs provide support for prenatal, maternal, and child health, high-risk families, perinatal depression, safe sleep, lactation support, child development, early learning and literacy, and health referrals. The target population is pregnant women and children ages 0-3.
Maternity Care Coalition’s MOMobile® HFA program supports families by:
- Providing intensive home visiting services to at-risk families
- Promoting healthy childhood growth and development
- Enhancing family well-being by reducing risks and building protective factors
MCC’s community health workers (who we call Advocates) support caregivers in establishing strong relationships with their children; provide support and education on parenting practices; address the physical and mental health of caregivers and children; promote healthy child growth and development through activities, education, assessments, and referrals; promote early learning by modeling age-appropriate activities; and promote early literacy and development through the utilization of evidence-based curricula.
With its long history of providing services to at-risk families and operating a successful HFA program, MCC will continue this necessary work in support of improved outcomes for children and families. MCC’s mission is to improve the health and well-being of pregnant women and parenting families and enhance school readiness for children 0—3. We envision a future where parents impacted by racial and social inequities can birth with dignity, parent with autonomy, and raise babies who are healthy, growing, and thriving.
