Healthy Families America (HFA) Advocate III

MCC seeks a full-time Case Manager responsible for providing in-home case management services to pregnant and newly parenting women including: assessment, referral, follow-up, monitoring and emotional support.   The Case Manager screens women for depression; helps clients access and utilize all necessary health and human services; helps women maintain their families’ primary health care; maintains careful records of all client contacts both in a paper file and in an online data base.

This position is a temporary position located in Delaware County serving Philadelphia.

Associate’s degree required, Bachelor’s degree in social work, human services, counseling, public health or closely related field preferred. We seek an individual with excellent organizational, time management, and people skills.  Must have strong oral and written communication ability; problem solving skills; knowledge of community resources; experience with children, and an interest in maternal and child health. Valid driver’s license with clean driving record required.

Healthy Babies Advocate

MCC seeks a self-motivated individual to fill the Advocate positions for a new program, Healthy Babies. This program will work with babies and caregivers, during NICU stays for Neonatal Abstinence Syndrome (NAS) due to fetal exposure to opioids or other substances, and after hospital discharge. Will be providing support to Philadelphia, Montgomery, Bucks and/or parts of Delaware County, requiring significant local travel. Qualified candidates will have experience living in Philadelphia communities served by MCC programs; knowledge and experience of substance use; knowledge of maternal child health or other community health issues; experience providing services in a community setting; and excellent communication skills. An Associate’s degree is required. A Bachelor’s degree in social work or a closely related field is strongly preferred. Candidates must have a valid driver’s license with clean driving record.

MOMobile Managed Care Advocate II

The MOMobile Managed Care Program seeks a Case Manager responsible for providing culturally sensitive home visiting, health education and case management services to pregnant and newly parenting women including: assessment, referral, follow-up, monitoring and emotional support.

Qualified candidates will have knowledge of maternal child health or other community health issues; experience providing services in a community setting; and excellent communication skills. Valid driver’s license required and Bachelor’s degree in social work, public health or related field preferred. ETO Database Administrator

The Program Evaluation Department seeks an energetic professional with excellent quantitative and database management skills to maintain and manage the agency’s performance management system, Efforts to Outcomes (ETO). The Administrator will be responsible for overall design, maintenance and management of ETO, training new staff and providing user support. The position also entails overseeing all staff data entry.

Qualified candidates will have a Bachelor’s Degree or an Associate’s degree with relevant experience. At least 1-2 years of experience in data management and program evaluation required. Experience in ETO preferred. In addition you must be customer-service oriented and highly responsive to program staff’s needs.

Riverside Program Coordinator

MCC seeks a self-motivated individual to fill the Coordinator position in our prison-based program. The Coordinator will provide administrative oversight of Riverside services and collaborate with the Riverside Correctional Facility staff. The coordinator will also maintain a reduced caseload providing culturally sensitive group education and case management to pregnant and newly parenting incarcerated and formally incarcerated women. Services will be provided at the Riverside Correctional Facility and during home visits. The successful candidate will also provide doula or labor support services on an on-call basis. Interest in maternal and child health and ability to work in a team environment desired. Master’s degree in social work or related field preferred. Bachelor’s degree required. Valid driver’s license with clean driving record required.

Safe Start Case Manager

MCC seeks a self-motivated individual to fill an Advocate position within the Safe Start MOMobile program. The Advocate will provide culturally sensitive in-home case management and patient navigation services to medically high risk pregnant women, including assessment, referral, follow-up, monitoring and needed supportive services. In this role, Advocates receive in-depth community health worker and doula training, among other trainings.

Qualified candidates will have experience living in Philadelphia communities served by MCC programs; knowledge of maternal child health or other community health issues; experience providing services in a community setting; and excellent communication skills. An Associate’s degree is required. A degree in social work, counseling, public health, nursing or a closely related field is a plus. Valid driver’s license with clean driving record required.
