Safe Start Case Manager

MCC seeks a self-motivated individual to fill an Advocate position within the Safe Start MOMobile program. The Advocate will provide culturally sensitive in-home case management and patient navigation services to medically high risk pregnant women, including assessment, referral, follow-up, monitoring and needed supportive services. In this role, Advocates receive in-depth community health worker and doula training, among other trainings.

Qualified candidates will have experience living in Philadelphia communities served by MCC programs; knowledge of maternal child health or other community health issues; experience providing services in a community setting; and excellent communication skills. An Associate’s degree is required. A degree in social work, counseling, public health, nursing or a closely related field is a plus. Valid driver’s license with clean driving record required. HFA Case Manager | North Philadelphia

MCC seeks a full-time Case Manager responsible for providing in-home case management services to pregnant and newly parenting women including: assessment, referral, follow-up, monitoring and emotional support.   The Case Manager screens women for depression; helps clients access and utilize all necessary health and human services; helps women maintain their families’ primary health care; facilities educational activities that support healthy child growth and development,  bonding and attachment; maintains careful records of all client contacts both in a paper file and in an online data base.

This position is located in North Philadelphia.  Bilingual English/Spanish Candidates are strongly encouraged to apply.

Associate’s degree required, Bachelor’s degree in social work, human services, counseling, public health or closely related field preferred. Early Head Start Advocate

An Advocate is needed for The Early Head Start program to conduct home visiting with pregnant women and families with young children ages 0 – 3 to provide health education, connect them to needed health care and other community resources and promote child development. The Advocate maintains relationships with identified clients and provides support via weekly home visits. They will also attend community meetings and trainings, keep accurate written records; and uses organization database. We seek motivated individuals, knowledgeable about community resources, with good computer skills. Ability to work well with people, good organizational skills. Valid driver’s license and clean driving record. High school diploma required. Associate/ Bachelor degree strongly encouraged.

Early Head Start Teacher

A teacher is needed for the Early Head Start program to assist with the implementation of the education program as outlined in both the adopted curriculum and the Head Start Performance Standards. The EHS teacher communicates daily with parents and staff regarding educational strengths and/or concerns of enrolled children, writes daily lesson plans and individualized daily schedules for each child, records daily observations and conducts quarterly home visits.

We seek interested individuals with experience working with young children (preferably infants or toddlers) and working with parents and community volunteers. A Bachelor’s degree is preferred, but an Associate’s degree in Early Childhood Education, CDA in Infant/Toddler is required. The position is in South Philadelphia. Individuals who are bilingual in Spanish/English and in Indonesian/English are encouraged to apply.

Early Head Start Custodian

MCC is currently seeking a full-time Custodian to support the South Philadelphia location. This position is directly responsible for cleaning and maintaining assigned areas of the facility, set up/clean-up for meetings and activities. Maintain site inventory for facility and other tasks as needed.

Qualified candidates will have at least 1-3 years of janitorial experience. Ability to lift and/or move 75 pounds. Operate common household cleaning appliances. They will maintain outside property including; snow, ice, and trash removal. Basic ability to troubleshoot and support maintenance issues preferred (plumbing and electrical). A High School Diploma is preferred.
